A few points, the first ridiculously simple:

1 When a team mate crosses in front of you, don't continue blazing away. Take your finger off the trigger and let them get bye. That way, you see, you don't take them down to half health or less. Besides, you need to practice firing in short bursts as it improves your accuracy and conserves ammo.

2 When a team mate is in a fire fight, move to one side and provide crossfire on the target. When your lined up, the enemy has a better chance of hitting one of you. When you are to one side, you are less likely to hit your team mate.

3 If you see that a teammate is hacking a deployable, don't throw a grenade or fire a rocket at it, move on!

4 When you're in an objective zone with teammates scattered about engaging the enemy, don't throw or drop a hand grenade. By the time it goes off things will have changed and you might blow up a team mate, or get killed before you can throw it which can do the same thing. Just take your rifle out and shoot!

this is one of the most important tactics, im surprised you just briefed it:p

1. when leaving a small chokepoint, like doors, hug the bloody wall. you will not get fired at by your teammates, and the enemy will have to focus outside the chokepoint (where it is easy to get kills) to get to you.
2. You can also run to the nearest cover, a lot of people will try to hunt you down, abandoning their positions. A good example is the island 1st phase, first square room you enter from front, before the generator. There is a crate there on the left, to which you can run to, and force the stroggies to scatter around.
3. Dont abandon your crossfire position, if there are more enemies coming. Throwing +20 health might help, but not when your not able to position yourself again.
4. Dont scatter the crossfire too much, a teammate must be able to get to you for a quick revive.
5. Dont keep it too tight, your a nade magnet.
6. Change your crossfire positions, so that a nade coming at a bad moment wont put you out of game.
7. People tend to choke well-setup crossfires. Push out whenever you can, to cause chaos and restart the choke. Each choke will eventually get overwhelmed, if a whole enemy team puts its mind to it.
8. Your crossfire position should cover at least 2/3 of all the entrances. 2 guys guarding one entrance each are in problem, because its not a crossfire but double duel. 3 guys, guarding 2 entrances each is a crossfire- you have double to firepower on all chokepoints, and the enemy usually can only get one of yours.

Biggest one IMO is when starting firing, dont forget to stop. So many times the enemy dies and team still sprays ammo, hitting their own medics who run out to revive. Also, in some cases its better to continue firing even if a member wanders into your line of fire. Its better to end up with 1 kill and 1 tk than 2 kills by the enemy. Besides, any more or less experienced player wont do it, unless its a real emergency, even then hed use quickchat.

I find this is very rare... You may be running out into the open too soon. :P
