Had this issue myself and figure there's got to be a few others with the same problem.
When running multiple monitors nVidia drivers switch the Manage 3D Setting "Multi-display/mixed-GPU acceration" setting to multiple montiors, it needs to be switched to single. Note this is only the newer drivers where they moved everything to a control panel inside explorer. For older drivers, I have no clue :P
It was impossible to navigate menu's with it like this, but this sorted. I believe this is also the same for Q4 / D3.
